What you'll do

• Observe care coordination and case management for clients up to age 21
• shadow Care Coordinators
• participate in supported service planning and care plan monitoring
• observe and assist with referrals and community resource connections
• learn documentation and confidentiality standards
• attend meetings and training
• observe crisis escalation and transition planning.
