Hospice Field Registered Nurse (RN)

What you'll do

• Conduct initial and ongoing comprehensive assessments
• determine hospice admission and recertification eligibility
• develop, implement, and document care plans
• educate patients and families about disease processes, self-care, end-of-life care, and nutrition
• determine service needs based on acuity
• direct hospice aides and LPNs/LVNs
• prepare clinical and progress notes
• report changes and outcomes to physicians and other personnel
• perform assigned on-call responsibilities.
