What you'll do

• Assist with residents’ psychosocial needs
• collaborate with the interdisciplinary team to develop and carry out individualized care plans
• support residents in achieving optimal health and well-being and maximizing quality of life
• apply knowledge of discharge planning, Medicare and Medicaid, OBRA regulations, and community resources.
