What you'll do

• Identify youth for program referrals
• review case records and interview youth and relevant contacts
• use online tools to identify connections
• prepare adoption-readiness assessments and recruitment plans
• meet monthly with youth and caseworkers
• engage family members and supportive connections
• coordinate pre-placement visits and transition plans
• support placement and adoption finalization
• attend training and submit monthly database data.
