MDS Coordinator

Job overview

The MDS Coordinator role is onsite in the Dayton, OH area and is full-time. Compensation is $85,000–$95,000 per year. The position supports a skilled nursing facility’s leadership team and works closely with the Director of Nursing and interdisciplinary team. The coordinator manages the Minimum Data Set assessment process so assessments and care plans are accurate, timely, and ready for reimbursement. The role also supports regulatory compliance, resident-status documentation, quality measures, and interdisciplinary care planning.

What you'll do

• Complete and submit MDS assessments and care plans accurately and on schedule
• coordinate with nursing, therapy, and social services to capture resident status
• monitor Patient Driven Payment Model accuracy and Quality Measures
• ensure compliance with state and federal MDS regulations
• lead interdisciplinary care-plan meetings. Requires an active Ohio RN or LPN license, MDS experience, and skilled nursing facility or long-term care experience.
