Hospice Registered Nurse Case Manager

The Hospice Registered Nurse Case Manager role is based in San Antonio, TX, with care delivered in patients’ homes and local travel required. The stated salary range is $70,000–$80,000 per year. The role reports to the Director of Nursing or Director of Patient Services and coordinates care with the interdisciplinary team, patients, families, and referring agencies. The nurse assesses patients’ clinical, functional, psychosocial, and environmental needs, implements and updates individualized plans of care, and educates patients and families about disease processes and end-of-life care. The case manager coordinates services, monitors responses to care, and performs assigned on-call responsibilities for patients and families.

• Conduct initial and ongoing comprehensive assessments

• communicate changes and outcomes to physicians and other personnel
• assess caregiver capacity
• determine service scope and frequency
• apply hospice admission and recertification criteria
• provide on-call service as assigned.
