What you'll do

• Conduct community outreach
• build relationships and assess client needs
• assist with public-benefit applications
• provide advocacy and transportation assistance for entitlement appointments as required
• obtain and track physician authorizations and reauthorizations
• attend referral-status meetings
• monitor hospitalizations and discharges
• coordinate care and referrals with providers, case managers, and agencies
• log interactions and maintain entitlement, recertification, and project records.
