Registered Field Nurse

Job overview

The Registered Field Nurse role is onsite, serving selected territories in Osceola County and Brevard County, FL, with pay of $1,500–$2,000 per week. Wheatland Health Community Care helps patients remain safe and independent at home through personal, companion, and memory care. The nurse completes home-based skilled visits and assessments, implements and updates care plans, educates patients and families, and documents care in the EMR. Coordination with physicians and care teams and early escalation of changes or safety risks support quality outcomes and help prevent avoidable hospitalizations.

What you'll do

• Complete start-of-care visits, recertifications, routine skilled visits, and discharges
• perform comprehensive assessments
• implement and update plans of care
• teach patients and families
• document assessments, interventions, orders, and signatures in the EMR
• coordinate with physicians, therapists, care coordinators, and administrators
• escalate changes in condition or safety risks
• participate in quality feedback.

Why this role
