What you'll do

• Assist with management of the Social Services Department
• encourage residents through counseling and other interventions
• implement person-centered care plans and monitor follow-up
• counsel residents and families on psychosocial, adjustment, and behavioral issues
• participate in Behavior Management Team meetings and interdisciplinary care conferences
• assist with resident discharge planning. The program requires completion of the facility-sponsored Social Service Designee program and continued social work education.
