Spiritual Care Counselor, PRN

The Spiritual Care Counselor, PRN role is onsite in Raleigh, NC. Compensation is not specified. The role assesses spiritual needs, develops care plan components with patients, families, facility staff, and the interdisciplinary group, and provides and documents counseling and spiritual care interventions. It also supports hospice staff and helps coordinate referrals to community clergy when requested.

What you'll do

• Assess and document spiritual needs
• collaborate on culturally sensitive care plans
• provide and document spiritual care and counseling interventions, including relevant sacraments and rituals
• provide end-of-life information and education
• refer patients to community clergy when appropriate
• support hospice and facility staff
• participate in performance improvement, orientation, and training
• maintain confidentiality.
