What you'll do

• Complete nutritional assessments and develop individualized nutrition care plans
• provide evidence-based nutrition education to residents, families, and staff
• collaborate with interdisciplinary team members to integrate nutrition into treatment plans
• monitor progress and adjust plans
• support dietary staff with menu planning, therapeutic diets, and regulatory requirements
• lead or participate in wellness education, group sessions, or in-service training
• document care using appropriate technology
• stay informed on nutrition research and best practices
• ensure compliance with federal, state, and facility guidelines. Full-time coverage is shared between two assigned facilities
• part-time coverage is 16 hours per week.
