Description

Individualized Planning & Service Oversight. Develop, review, and approve Individualized Service Plans (ISPs) in collaboration with individuals served, families, case managers, and interdisciplinary team members. Ensure ISPs are person-centered, outcome-driven, and aligned with assessed needs, strengths, and preferences. Monitor ISP implementation to confirm services are delivered as written.
