What you'll do

• Complete the Resident Assessment Instrument process and care-management process from admission through discharge
• coordinate assessment schedules and care-plan reviews
• monitor MDS and care-plan documentation
• code and submit resident assessment data
• coordinate interdisciplinary meetings and daily stand-up reviews
• support staff instruction on RAI, reimbursement, and clinical computer systems
• maintain confidentiality.
