What you'll do
- Conduct initial and ongoing comprehensive assessments
- determine admission and recertification eligibility
- develop, implement, and document plans of care
- communicate with patients, families, team members, and healthcare professionals
- apply infection-control precautions
- educate patients and families on disease processes, self-care, end-of-life care, and nutrition
- determine service scope and frequency
- initiate preventive and rehabilitative nursing procedures
- provide clinical direction to home health aides and LPN/LVNs
- prepare clinical and progress notes
- report changes in patient needs and outcomes
- monitor cases for third-party payer compliance.
