What you'll do

• Conduct bio-psycho-social assessments
• develop individualized service plans focused on housing stability and well-being
• provide case management related to daily living, crisis intervention, finances, mental health, substance use, and benefits
• coordinate with treatment providers
• connect tenants with community resources and supportive services
• provide individual and group therapeutic interventions
• document assessments, plans, interventions, and progress.

Why this role

The schedule is onsite Monday–Friday: 9 a.m.–5 p.m. on Monday, Wednesday, and Friday, and 10 a.m.–6 p.m. on Tuesday and Thursday. A master’s degree in a listed or related field is required.
