Targeted Case Manager, CCBHC

Job overview

The Targeted Case Manager role is onsite in Lockport, NY, with a Monday–Thursday schedule of 8:00 a.m.–5:15 p.m. and Friday hours of 8:00 a.m.–1:00 p.m.; pay is $20–$24 per hour. The role supports patients in Horizon Health Services’ behavioral health and addiction services. The case manager helps patients access timely care and address barriers by connecting them with medical, social, legal, housing, and other community resources, including during transitions from inpatient, emergency, or residential treatment.

What you'll do

• Complete Social Determinants of Health assessments
• participate in Open Access
• support patients through outreach calls, telehealth, and face-to-face visits
• provide transitional care follow-up
• connect individuals with community services
• help address care gaps and health goals
• use clinical and non-clinical data from EHR systems and regional health information networks
• maintain privacy and confidentiality.
