The Certified Occupational Therapy Assistant / Home Care Coordinator role is an onsite position in Bedford, PA, with compensation of $27.60–$32.50 per hour. Community LIFE serves nursing-home-eligible adults aged 55 and older living in the community and works to help older adults remain at home. The coordinator assesses participant needs and arranges appropriate home and personal care in collaboration with the interdisciplinary and therapy teams. The role supports rehabilitation potential, continuity during transitions after hospitalization or skilled nursing stays, and efforts to minimize or avoid inpatient care.

What you'll do

• Assess participant needs and coordinate participant-centered home and personal care services
• collaborate with the interdisciplinary and therapy teams to formulate in-home care plans
• support transitions to and from home after hospitalization or skilled nursing care
• participate in weekend and on-call rotation.
