What you'll do

• Conduct home and field visits to assess children’s physical, social, motor, cognitive, and language development
• use developmental and therapeutic assessment activities
• consult with health care providers and educators
• assess developmental milestones for children in the household
• collaborate with care management staff on individualized service plans
• monitor progress
• provide parenting skills training and caregiver counseling
• develop community resources
• provide crisis intervention as needed
• maintain electronic case files and required charts and data
• use trauma-informed and culturally responsive approaches
• staff a rotating emergency on-call phone.
