What you'll do

• Connect families with post-permanency services
• collaborate with foster care and WWK staff
• implement the Family Adoption Preparation curriculum
• complete family and diagnostic assessments
• develop service and treatment plans
• provide bonding activities, parent coaching, TBS/CPST, and educational advocacy
• make referrals
• coordinate providers and family events
• support virtual groups and social media
• collect program data and complete required records. Participate in a rotating after-hours cell-phone on-call schedule for consultation and crisis response.
