The Hospice On-Call Registered Nurse (RN) role is onsite, serving the Pilot Point, Denton, and Sherman area in Texas. Compensation is not specified. Concho Hearts Hospice provides compassionate care in patients’ homes and support for patients and families facing end-of-life illness. This full-time, on-call-only role exists to provide nursing care in patients’ residences and coordinate care with the interdisciplinary team, patients, families, and referring agencies. The nurse supports symptom management, care planning, patient and family education, and continuity of care during a seven-days-on/seven-days-off call cycle.

What you'll do

• Administer nursing care to patients in their homes and coordinate care with the interdisciplinary group, patient, family, and referring agency.
• Prepare and adjust patient care plans as needed.
• Provide caregivers with patient-care information, end-of-life education, and emotional support.
• Maintain and update medical charts, patient files, and health records.
• Respond to PRN emergency calls and travel locally as needed.

Why this role

On-call-only full-time position with a seven-days-on/seven-days-off call cycle. Requires current RN licensure or eligibility under Nurse Licensure Compact rules, a current driver’s license, CPR, reliable transportation, and current auto liability insurance. Two years of clinical RN experience is preferred; hospice experience is preferred.
