Hospice Community Liaison

Job overview

The Hospice Community Liaison role is onsite in Albuquerque, NM, with a Monday–Friday schedule and occasional evenings and weekends for community events and referral-source availability. Pay is $85,000–$110,000 per year, plus performance bonus. LilyCare Hospice serves patients and families across greater Albuquerque in homes and care facilities.

What you'll do

• Build and deepen referral relationships with hospitals, skilled nursing facilities, assisted living communities, and physician practices
• plan territory prospecting and account development
• deliver in-services and community education
• manage the marketing budget
• document activity in the CRM
• report account progress
• and coordinate referral handoffs with the clinical team.
