Community Health Navigator (Contract)

What you'll do

• Educate community members about Medicaid redetermination timelines and requirements
• assist with forms and documentation
• connect residents with Cook County Health services
• provide in-person resource navigation
• attend outreach events across Cook County, including evenings and weekends
• build relationships with community partners
• distribute educational materials
• maintain interaction and referral records
• participate in team meetings, training, and quality improvement.
