The Case Manager – Rapid Response Team – Swing Shift role is onsite in San Diego, CA. Compensation is $23–$28 per hour. PATH works to assist people experiencing homelessness through outreach, case management, supportive services, and housing programs. The case manager helps participants address housing and other personal goals through individualized assessment, service planning, referrals, and ongoing review. The role supports clients across stages of homelessness and connects them with resources to help move toward stability.

What you'll do

• Conduct intake and individualized needs assessments
• collaborate with participants on service plans addressing housing, income, and other identified goals
• provide referrals, linkages, information, and support
• complete program-specific assessments
• evaluate strengths and challenges
• conduct a 90-day review after initial intake.
