What you'll do

• Complete comprehensive biopsychosocial assessments
• coordinate social services and counseling support
• monitor and document progress toward service plan goals
• refer families for counseling or other services
• collaborate with case managers and support staff
• improve connections to mental health, education, and community services
• maintain records and evaluate service effectiveness
• conduct staff trainings and resident workshops
• complete supportive housing applications as needed.
