What you'll do

• Provide hospice nursing care in patients’ residences and coordinate with the interdisciplinary team, patients, families, and referring agencies
• assess disease impact and eligibility
• develop and document care plans
• educate patients and families
• determine service needs
• apply infection-control precautions
• direct hospice aides and LPNs/LVNs
• document progress
• communicate changes to physicians and care personnel
• monitor assigned cases
• participate in quality programs
• and perform assigned on-call responsibilities.
