What you'll do

• Assist with collecting social history data and developing discharge plans, MDS, and care plans for new admissions as directed
• orient new residents and help residents and families adjust to changes, including room changes
• assist families with marking residents’ personal items
• help facilitate family group meetings
• document progress notes related to resident care plans within policy timeframes
• participate in the departmental budget and provide a monthly supplies report to the Administrator
• perform additional assigned duties.

Why this role
