What you'll do

• Assess, plan, implement, and evaluate nursing care
• provide hospice care focused on pain control and symptom management
• coordinate with physicians, hospice staff, agencies, and the interdisciplinary team
• maintain patient records and documentation
• administer medication in accordance with laws and agency policies
• assess hospice eligibility with the Medical Director
• counsel patients and families on end-of-life needs
• participate in on-call duties.
