What you'll do
- Conduct and coordinate MDS assessments and care plans
- notify care team members of due dates and ensure their portions are completed
- develop or revise care plans quarterly and after significant changes
- complete required PPS and state assessments, including MDS, CATs, and CAAs
- coordinate care plan meetings
- submit MDS information to state and federal agencies on time
- maintain assessment schedules
- protect resident privacy and confidentiality
- communicate changes in resident condition across departments
- monitor resident care during the MDS process and report concerns
- assist with pre-admission screening as needed.
