What you'll do

• Conduct consultations to understand client needs and guide families toward care decisions
• coordinate service starts and onboarding
• maintain ongoing engagement, reassessments, and care plan adjustments
• partner with scheduling, operations, and Care Pros
• manage portfolio data and documentation
• support referral relationships and local market reputation as needed.
