The role coordinates with providers and community partners to support continuity of care for people with complex behavioral health needs.

What you'll do

• Accompany law enforcement on co-response calls as directed
• provide initial behavioral health support and de-escalation
• intervene during mental health or substance use crises
• complete situational assessments and referrals
• develop stabilization plans with clients and families
• coordinate services and follow-up
• document contacts and outcomes
• collaborate with community partners.
