Remote Registered Nurse, Admission and Discharge Care Coordination

The Remote Registered Nurse, Admission and Discharge Care Coordination role is fully remote for Illinois. Compensation is $50–$65 per hour. The role helps members navigate care, understand follow-up and medication guidance, and connect with providers and community resources to support quality outcomes and reduce avoidable readmissions.

What you'll do

• Coordinate discharge care and episodic case management
• provide pre-admission and post-discharge counseling
• consult with physicians, providers, members, and discharge planners
• assess and coordinate services
• educate members on medication safety and post-operative instructions
• support follow-up appointments and referrals
• assess case complexity and refer cases as needed.
