What you'll do

• Visit patients at skilled nursing, senior living, and hospital facilities
• explain services to patients and caregivers
• educate patients about remote monitoring
• complete enrollment and required information
• coordinate with facility and hospital staff, case managers, discharge planners, and TeleMate colleagues
• help patients understand equipment and next steps
• communicate needs and barriers
• provide clinical support to the monitoring team
• document interactions
• conduct appropriate outreach and follow-up.
