What you'll do

• Complete holistic patient assessments
• recognize and report abnormal findings and changes in condition
• coordinate and revise individualized plans of care with patients, families, and the multidisciplinary team
• communicate using SBAR and handoff processes
• initiate discharge planning and referrals
• provide evidence-based nursing care and patient/family education
• respond to emergencies
• follow infection prevention and safety practices
• evaluate care against standards and patient outcomes.
