What you'll do

• Provide case management and monitor active treatment
• train and consult with foster parents
• conduct and document scheduled and unscheduled home and community visits
• assess home quality and compliance
• support individualized schedules and goals across educational, emotional, independent-living, medical, relationship, and social domains
• prepare monthly summaries
• complete assessments
• monitor records and compliance
• participate in team meetings and hearings
• communicate with foster parents, state monitors, schools, and other partners
• maintain required annual training
• respond through ongoing on-call status to urgent or emergent situations.
