What you'll do

• Educate individuals about managing mental illness and/or substance use disorders
• provide transportation when medically necessary and appropriate
• connect people with community resources and natural supports
• help develop interpersonal, social, family, community, and independent living skills
• complete documentation promptly
• follow evidence-based practice models and agency policies
• apply working knowledge of diagnostic criteria
• support program compliance with CARF and DMH standards and the agency strategic plan.
