Facility Navigator

The Facility Navigator role is onsite in Cocoa, FL; compensation is not specified. The navigator serves as the main point of contact between a designated skilled nursing facility and the group. The role coordinates communication and documentation among facility staff, providers, patients, and support teams to help maintain accurate records, continuity of care, and appropriate follow-up.

What you'll do

• Communicate regularly with facility staff, clinical management, rounding providers, schedulers, medical record specialists, and case management
• support visit documentation and patient record updates
• assist with Annual Wellness Exam and MIPS questionnaires
• capture vital signs as needed
• follow prescription refill procedures
• monitor and report acute abnormal findings
• help ensure diagnostic information is available to providers
• respond appropriately to urgent or emergent situations
• follow HIPAA confidentiality standards.
