The Home Health Clinical Case Manager (RN) is an onsite, full-time role in Chicago, IL, supporting Safe Life Home Health Care’s Chicago and Lombard branches. Pay is $37–$42 per hour. What you'll do

• Review and triage home health referrals
• assess appropriateness based on clinical needs, payer requirements, service area, and staffing
• coordinate documentation with referral sources and internal teams
• communicate with patients and families
• track pending referrals and missing records
• prioritize urgent cases
• support case assignment and admission readiness
• document in the EMR
• help resolve referral barriers
• support Chicago and Lombard branch operations.
