Home Health Registered Nurse is an onsite role providing care in patients’ homes in Stockton, CA. Compensation is $70–$120 per hour. The role is part of ACAVAM Home Health, the adult home health division of Delta Bay Healthcare. The nurse assesses patients, develops and updates individualized care plans, administers prescribed treatments, and educates patients and families. By coordinating with physicians and other care professionals and monitoring progress, the role supports health outcomes and independent living.

What you'll do

• Assess patients in the home
• develop, implement, and update individualized nursing care plans based on assessments and physician orders
• administer prescribed medications, treatments, and therapies
• educate patients and families on disease management, medication, wound care, and prevention
• coordinate with interdisciplinary teams
• document care and observations
• monitor progress and adjust plans
• provide emotional support and counseling.
