Hospice Field Registered Nurse (RN) is an onsite field role based in Conroe, TX, providing care in patients’ homes or other care settings. Compensation is not specified. What you'll do

• Conduct initial and ongoing comprehensive assessments
• apply hospice admission and recertification criteria
• develop, implement, and document plans of care
• educate patients and families about disease processes, self-care, end-of-life care, nutrition, and dietary needs
• determine service scope and frequency
• assess caregiver capacity
• direct hospice aides and LPN/LVNs
• prepare clinical and progress notes
• communicate changes and outcomes to physicians and other personnel
• perform assigned on-call responsibilities.
