Hospice Triage Registered Nurse (RN) — Nights is an onsite field role based in Escondido, CA, serving San Diego County and South Riverside. The schedule is four 10-hour shifts per week, 9:30 p.m.–8:00 a.m.; pay is $45.00–$61.00 per hour. The Elizabeth Hospice’s mission is to enhance quality of life for people nearing the end of life and those who grieve. The nurse provides timely hospice assessments, visits, and care to support patient comfort and family needs. The role responds to visit requests, manages symptoms and ordered care, educates patients and caregivers, documents care, coordinates with clinical colleagues and physicians, and directs and supervises LVN and home aide services according to care plans and policy. Two on-call shifts are required monthly, and travel is required for patient visits.

What you'll do

• Perform visits as directed and respond promptly to patient visit requests
• assess immediate patient and family care and symptom-management needs
• provide ordered care and health instruction
• follow up with case managers and team members about changes
• direct and supervise LVN and home aide services under the care plan and policy
• maintain timely electronic point-of-care documentation
• collaborate with physicians and supervisors on orders and level-of-care changes.
