Provide physical therapy services according to the written physician’s plan of care and the state Physical Therapy Practice Act. Assess and evaluate therapeutic, rehabilitative, and functional status; participate in developing the total plan of care. Evaluate the home environment to support patient safety and comfort. Assess muscle strength, mobility, gait, range of motion, and transfer abilities—
