Registered Nurse (RN) Case Manager – Hospice

The Registered Nurse (RN) Case Manager – Hospice role is field-based in Westchester, IL, with a Monday–Friday schedule from 8:30 a.m. to 5 p.m. Pay is $75,000–$90,000 per year. Moments Hospice provides comfort-focused care and support to patients and families at the end of life. The RN serves as the clinical lead for an assigned caseload, assessing needs, managing symptoms, and coordinating individualized care with patients, families, physicians, and the interdisciplinary team. The role also educates caregivers and documents clinical interventions to support compassionate, coordinated hospice care.

What you'll do

• Complete initial and ongoing physical, emotional, and psychosocial assessments
• develop and update individualized plans of care with patients and families
• provide skilled nursing, pain and symptom management, medication administration, and comfort measures
• educate patients and caregivers
• coordinate care and serve as liaison among patients, families, physicians, and the care team
• participate in interdisciplinary group meetings.
