Registered Nurse — Non-Medical Home Care Quality and Innovation (PRN)

The nurse supports BarCARE’s non-medical home care services and its efforts to strengthen client safety, caregiver education, quality improvement, and care coordination. The role conducts assessments and supervisory visits, supports caregiver training, and helps improve the processes that enable safe, accountable home care. The nurse identifies and communicates concerns, supports follow-through, and contributes to new quality and safety initiatives while respecting the distinction between non-medical care and skilled nursing.

What you'll do

• Conduct new-client assessments and reassessments
• develop and update individualized non-medical care plans
• document services in the agency’s electronic system
• conduct caregiver supervisory visits and provide coaching and feedback
• assist with caregiver orientation, competency, and annual training
• educate caregivers on safety, infection control, documentation, and reporting changes
• communicate and follow up on client-safety, condition, performance, and service-delivery concerns
• review records for accuracy and compliance
• participate in quality assurance and performance improvement
• help implement and evaluate client-safety, caregiver-support, and care-coordination initiatives
• travel throughout assigned service areas as needed.
