What you'll do
- Complete and document social work and psychosocial assessments within three days of admission or readmission
- ensure required forms are accurate
- schedule and coordinate resident care conferences
- complete MDS sections and applicable CAAs
- develop and update care plans
- monitor behavior logs and high-risk residents
- attend Medicare and QAPI meetings
- coordinate with psychiatric and ancillary providers
- educate residents and families about advance directives
- document interactions
- support adjustment to the facility
- coordinate hospice referrals when indicated
- begin discharge planning on admission
- provide discharge-rights information
- arrange durable medical equipment and home care at discharge
- and maintain community relationships.
