What you'll do
- Participate in discharge planning, social care plans, resident assessments, and interdisciplinary meetings
- coordinate safe patient transitions and communicate with patients and responsible persons
- provide information about Medicare, Medicaid, and financial assistance
- consult with staff on psychosocial concerns and placement
- contribute to quality programs and grievance tracking
- and maintain confidentiality and required documentation.
