Hospice Chaplain

What you'll do

• Assess spiritual needs and ensure timely services
• participate in comprehensive assessments and individualized care planning
• provide direct spiritual care
• liaise with community chaplains and spiritual counselors
• document services and communications
• attend patient care conferences and interdisciplinary meetings
• support education and recruitment of community spiritual care resources
• participate in quality improvement
• provide requested funeral or memorial services and plan periodic memorial services.

Why this role

Requires a degree from an accredited seminary or school of theology (BD, MDiv, or equivalent), experience working with death and dying, comfort in interfaith settings, and a valid driver’s license and insured automobile. Hospice experience is preferred.
