What you'll do

• Plan, organize, implement, evaluate, and complete social service requirements
• perform resident evaluations as required by state regulations
• support admission and retention requirements
• assist residents and families with transitions and facility orientation
• help residents and families access services
• participate in individual care plan development and implementation
• coordinate services with management
• document case management notes
• support discharge planning. Long-term care or sub-acute experience is required
• BSW or a master’s degree in a related field is required
• certification or licensure applies as required by state regulations.
