What you'll do

• Conduct multidimensional assessments through home visits
• assess ongoing needs through client, collateral, and informal-support contacts
• develop and adjust service plans
• consult with Senior Care Options personnel and coordinate approved services
• monitor service quality
• maintain complete and timely client records
• advocate for clients and identify community resources
• participate in team activities and provide outreach as a Boston Senior Home Care representative.
