What you'll do
- Review referrals and respond promptly
- approve routine admissions within facility guidelines and escalate unusual or high-risk cases
- obtain missing clinical, insurance, and authorization records
- track referrals through admission or closure
- communicate decisions and requirements
- conduct bedside evaluations, tours, hospital visits, and community outreach
- coordinate transitions and maintain referral records
- report updates and trends to facility leaders.
