What you'll do

• Assess patients during visits and document observations
• take vital signs and review medications
• complete OASIS assessments
• provide wound care and IV therapy
• educate patients and families on home health care
• coordinate with occupational, physical, and speech therapists, physicians, and other care providers
• recommend devices or tools that may improve quality of life
• respond to patient concerns and requests.
